Join us and you’ll feel the support of an entire team that understands the full picture of your health. We’ll work together with you to create a comprehensive plan that’s tailored to your individual needs. Our Care Team Model helps patients achieve:
Better outcomes
Patients who receive care from a primary care patient care team have been shown to have better health outcomes, including lower rates of hospitalization and fewer complications.
Patients are more satisfied with their care when they have a primary care patient care team, as they feel that their needs are being met and that they have a team of professionals who are working together to help them.
Patient-centered care means the focus is on you and your health journey.
Our primary care patient care team model offers a patient-centered approach to healthcare that focuses on you. Since everyone’s story is different, our doctors and staff take the time to listen to yours. Your personal care plan is completely customized to help you stay healthy.
Comprehensive and Coordinated Care
With a team approach, patients receive personalized care that is comprehensive and coordinated. Each team member has a specific role to play in the patient’s care, and they work together and meet regularly to ensure that the patient’s needs are met.
Patients have better access to care because there is an entire team available to meet each need whether it be a same-day appointment, answer a question, submit a prescription, or schedule the next appointment.
Enhanced Quality of Care
The team approach allows for multiple perspectives being shared among healthcare professionals, which can lead to better patient diagnosis and treatment.
Our team approach also reduces healthcare costs by preventing unnecessary hospitalizations and emergency department visits, improving chronic disease management, and promoting preventive care.

Primary Care Physician
Your doctor serves as the main point of contact for the patient and is responsible for coordinating the patient’s care.
Nurse Practitioner or Physician Assistant
NPs/PAs are trained to provide primary care services under the supervision of a physician. They often can spend more time with patients, which allows them to develop a rapport with patients and get to know them on a personal level. This knowledge helps them promote self-care and prevent complications, ultimately contributing to better patient outcomes.
Nurse
Nurses are essential in the coordination of patient care and in supporting physicians to deliver quality healthcare. They evaluate patient needs, monitor health status, and provide education to patients and their families about their health conditions, medications, and other aspects of care.
Medical Assistant
MAs play an important role in setting the tone for the patient’s visit and creating a positive overall experience. MAs perform a variety of administrative and clinical tasks that may include scheduling appointments, managing patient records, taking vital signs, assisting with medical procedures, and providing patient education.
PRCs help patients navigate the complex world of healthcare by providing support, education, and advocacy. By working closely with patients, providers, and billing, PRCs can help ensure that patients receive the care they need, when they need it, and without unnecessary financial burden.
On-site Lab Technician
On-site Lab Technicians play a vital role in providing accurate, timely, and convenient diagnostic testing; supporting effective patient care; and contributing to the smooth functioning of our primary care office. They work closely with our providers to foster collaborative relationships and facilitate better communication regarding patient diagnostic.
