When you make prescription requests by email, be sure to include your full name, medication requested, pharmacy name and telephone number, your physician's name and a telephone number where you can be reached. Just as with phone requests, please provide at least 24 hours notice. If you have a medical emergency, you may page the on-call physician through the main number. When you call, be sure to leave a clear message with your name and telephone number, including area code. The on-call physician will call you back within 30 minutes. Please do not use email to discuss your health status or seek medical advice. We care about your privacy and the confidentiality of your personal health information. Email communications should be limited to appointment scheduling, referrals, or billing inquiries. Holidays The office is closed on the following days: |
